Ancillary Insurance Coverage Employees interested in disability, cancer, or intensive care insurance may purchase this on their own, running the premiums through our payroll deduction program. The company offers this as an administrative service only to the employee. Inquires relating to any ancillary insurance should be directed to the insurance agent. Furthermore, the company makes no representations with respect to any ancillary insurance coverage and any administrative assistance provided shall not be construed as endorsing such insurance coverage. Dental/Optical Fund Our company establishes an account for each eligible full time employee and their dependents, which will serve as an allowance for family dental and optical expenses. All regular full-time employees who are eligible for the company health insurance plan are able to receive this benefit. Employees become eligible for this benefit at the beginning of the month following 60 days of continuous employment on our regular payroll. The benefit amount available is $200.00 per calendar year and may be carried over if not used up to a maximum of $600.00. The company will cover 50 percent of claims for dental visits, braces, and or eye exams, eyeglasses and contacts up to the balance of your account. Allocations made by the company will take place on January 1st every year. Dental or optical bills should be turned into the Accounting Department for reimbursement.

Direct Deposit - Payroll Siemer Milling Company and Hodgson Mill, Inc. are happy to offer Direct Deposit of your payroll check. Your payroll check can be divided between two different accounts (checking account and savings account) if you so desire. If you elect to have your paycheck split between two accounts, you must make sure that you indicate the percentage that you want deposited into your savings account. You may enroll for direct deposit at any time. Contact Human Resources for the necessary forms. Employee Assistance Program The Employee Assistance Program (EAP) provides confidential and professional assistance to help employees and their families overcome problems such as family or marital difficulties, financial and legal problems, emotional difficulties or problems caused by alcohol or drug abuse. The company pays for the first session. Any subsequent sessions are the financial responsibility of the employee. If the employee participates in the group health plan they could be eligible for financial assistance through that particular program also. Please talk with the Human Resources Department for details on this program.

Employee Discounts (Limited amount of free products) Employees are entitled to receive a limited amount (3 products) of free Hodgson Mill products on a weekly basis.
Employee Incentive Programs We provide a company incentive program called "Points for Performance", for all eligible employees. Please review the official program provided by your immediate supervisor or in the human resources department for complete details.
Employee Referral Program Our company recognizes the importance of good employees. It is well documented that the best candidates tend to be those recommended by current employees. Generally they fit the culture better because they are "pre-selected", they come in with a positive image of the company, there is greater peer pressure to perform, etc. Because of this, our company offers a referral bonus to all employees who "recommend" or "refer" a candidate that is hired by the company.

Flexible Spending Account Employees can save tax dollars by enrolling in this multi-option plan. This allows employees to withhold pre-tax dollars from their paycheck to pay:
1. Group Health Insurance Premiums 2. Medical reimbursement, which includes dental and optical expenses, out-of-pocket expenses such as meeting deductibles, etc. 3. Child or dependent care expenses. 4. Premiums for any qualified individual ancillary insurance you may have purchased.
The benefits eligibility date must have been reached for this benefit to become effective. Elections may only be changed during the annual election period, which is from November to December of each year. Allowable expenses include only those that were incurred during the plan year or calendar year in this case. Expenses may be submitted up to 60 days following the end of the plan year. Contributions not used will be forfeited in accordance with Federal Law.

Healthy Lifestyle Assistance Our company believes in helping employees maintain a healthy lifestyle. One of these programs is the cessation of tobacco use program started in 1992 helping employees to "kick the habit". The company will also refund the cost of nicotine patches for a continuous period not to exceed three (3) months. At the end of a nine (9) month period if the employee has ceased using tobacco for the entire nine (9) months, a cash reward of $250.00 will be given to the employee. If the employee has not ceased the use of tobacco for the entire nine (9) months, then the employee will be required to reimburse the company for the cost of the nicotine patches. This program is limited to full-time employees and doesn't extend to spouses of family members. Employees are permitted to participate in this program once. Holidays All full-time employees who have completed 60 days of continuous service, from their anniversary date, will be allowed up to eight hours pay at regular hourly rates for each of the following holiday's regardless of the days on which they fall:

If an eligible employee works on a recognized holiday, he or she will receive holiday pay plus wages at one and one-half times his/her straight time rate for the hours worked on the holiday. Salaried employees must schedule a day off in lieu of paid holidays.

Personal Days All full time and part time employees are eligible for up to four paid personal days per calendar year based on their average hours worked. All full-time employees receive up to four paid personal days at the beginning of the calendar year following their date of hire. New employees will receive prorated time of up to one personal day for every three months worked prior to the start of the next calendar year. An employee can elect to use this time in increments of four (4) or eight (8) hours.
Personal days awarded each calendar year may be used in any manner. Once the employee allotment of personal days has been used, no additional days will be given until the following calendar year. Personal days must be used during the calendar year as they will not be paid out or carried over to the next year.

Uniform and Uniform Maintenance We provide uniforms for all production employees. The company through the local uniform service provides these uniforms. Office personnel should dress in a businesslike manner. We want to uphold a professional and businesslike image for our customers. Uniforms are not intended for attire at home or leisure. Modifications such as cutting off the sleeves are not permitted. The company pays for cleaning and repair costs to uniforms.
Vacation Days The vacation period is based on an anniversary year; all full-time employees earn vacation time based on the following schedule:
After one anniversary year of service - 40 hours. After two anniversary years of service - 80 hours. After ten anniversary years of service - 120 hours.
Employees hired prior to 5/16/90 will be entitled to 160 hours of vacation following twenty (20) years of continuous employment from their anniversary date. No more than two weeks may be taken consecutively, without permission of management.
Vacation time can be used in minimum increments of four or eight hours. Unused vacation time at the end of an anniversary year will be forfeited. No vacation time may be carried over to the next year and the time not taken will not be paid out.
To take vacation, employees should request advance approval from their supervisors. Requests will be reviewed by a number of factors, including business needs and staffing requirements. In the event several requests for the same period are received, preference will be granted on the basis of seniority. The final decision, however, is left to the discretion of management.
